Researchers in another paused trial, called CATALYST, worry about a very specific risk. That study had enrolled more than 3000 women in South Africa, Zimbabwe, Lesotho, Kenya, and Uganda to compare different strategies to prevent HIV infection using what’s known as pre-exposure prophylaxis (PrEP). Some participants received an injectable PrEP drug, cabotegravir, which offers protection for 2 months. When those injections end, low amounts of drug linger in the body for up to 1 year and could foster the emergence of drug-resistant HIV strains that could then be transmitted to others.

To avoid this, people who stop taking the injections are asked to take PrEP pills for 1 year. It’s not clear whether women who received injected cabotegravir in CATALYST, which is run by the U.S.-based nonprofit FHI 360, will get access to those pills. “The stop work order has forced us to walk away without meeting our most basic ethical obligations to study participants,” says a staffer on the project.

Die Foundation for #AIDS Research amfAR schätzt, dass durch das abrupte Ende der Kostenübernahme für HIV-Medikamente in Afrika durch #PEPFAR in den nächsten 3 Monaten 137.000 Kinder mit HIV geboren werden. Durch Fortführung der Medikation für die Schwangeren würde das verhindert. Mit HIV geborene Kinder brauchen ein Leben lang Medikamente, um das Virus zu unterdrücken.

Weil auch die HIV-Tests nicht mehr durchgeführt werden können, wird man nicht wissen, ob ein Baby infiziert ist.

Based on an expected HIV transmission rate of 40% when a woman is not on treatment and assuming that 50% of pregnant women on ART would deliver during a 90-day stoppage, we estimate that this would mean 135,987 babies acquiring HIV. These babies, however, are highly likely to also go undiagnosed because infant HIV testing services are also being suspended due to the stop work order.
